The Nurse Practitioner on the Community Living Center (CLC) is a Masters prepared, nationally certified, highly skilled and experienced NP who provides primary care to (mainly) geriatric resident Veterans.  The CLC provides long term and short term care to Veterans who require skilled nursing care, dementia/psychiatric care, hospice/palliative care, or skilled rehabilitation in a home-like environment.  Duties include, but are not limited to:

  • Performs complete physical examinations, reviews medical histories, lab results and imaging studies to determine diagnosis of residents condition.
  • Document patients comprehensive health history in electronic medical record.
  • Prepare written consultations containing thorough and concise patient data.
  • Provides treatments and medications according to her/his scope of practice.
  • Provides care across the healthcare continuum with emphasis on the promotion of health, stabilization of the disease process,  prevention of complications and in the case of hospice/palliative care, pain control and symptom management.
  • Provide age-specific care with special expertise for the geriatric population, and those with chronic illnesses and end-of-life needs.
  • Collaborates with other members of the nursing staff, medical staff and other disciplines to promote the development of high standards of practice and optimum resident care.
  • Provide education to staff, residents and families as needed.
  • All other duties as assigned.

BASIC REQUIREMENTS:   

  • U.S. citizenship.  Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency.
  • Nurse Practitioners in the Department of Veterans Affairs must possess a Masters degree from a program accredited by the NLNAC or CCNE.
  • Nurse Practitioners must be licensed or otherwise recognized as a nurse practitioner in a State.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Possess prescriptive authority in NYS.

EXPERIENCE REQUIREMENTS: 
2-3 years of nurse practitioner experience that demonstrates leadership and performance in the application of the nursing process to patient care, organizational processes and/or systems and improving patient care outcomes.  NP experience in Adult health with emphasis in Hospice and Palliative care specialty area preferred.

GRADE DETERMINATIONS:
Nurse II - Masters degree in nursing with a BSN and approximately 1-2 years experience.

Nurse III - Masters degree in nursing with a BSN and approximately 2-3 years experience.

 

Applicants education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected.  Experience as a RN/NP will be evaluated to determine if the experience is of an acceptable level of quality with regards to the following four dimensions of nursing:

Physical Requirements:

Appointment to this position requires satisfactory completion ofpre-employment physical examination. The functional requirements for a NursePractitioner are: Moderate lifting, 15-44 pounds, light carrying, under 15pounds, straight pulling (2 hours), pushing (2 hours), reaching above shoulder,use of fingers, both hands required, walking (3 hours), standing (6 hours),kneeling (1.5 hours), repeated bending (2 hours), near vision correctable at13" to 16" to Jaeger 1 to 4, Far vision correctable in one eye to20/20 and to 20/40 in the other, hearing (aid permitted), clear speech) andworking closely with other.
