RN/Patient Care Coordinator at Eddy Village Green DayBreak in Cohoes, NY

**No weekends, shifts, or holidays!! Monday-Friday, 8am-4pm!

Our Eddy Cohoes DayBreak is looking for an RN who can join our wonderful team of caregivers in our uniquely special program. Our Medical Adult Day Program provides health care services and recreational activities to functionally impaired adults whie working with their caregivers and physicians in the community.
This role coordinates care and provides skilled nursing services while using strong assessment skills. It also involves making physician contacts, careplanning, ongoing tracking and monitoring of care, medical supervision of CNAs, and working with the social worker as part of a professional team. This position is a chance to really make a difference!!

Requirements:
-Strong computer skills.
-2+ years of experience in geriatrics and/or experience with functionally impaired adults.
-Current NYS RN license.
-Current CPR certification.
