Nurse Case Manager (RN Registered Nurse)Job Responsibilities
As a Nurse Case Manager your goal is to create an ICP that will help improve the health status, functioning, community integration and quality of life for members in their preferred place of residence. This involves meeting directly with members, their families and providers in office or home settings to review members’ functional status, medical resource needs, and individual care plans.
Additional responsibilities:
- Analyzing data and using it to improve care delivery
- Organizing primary care teams and acting as facilitator to ensure that members’ needs are met
- Preparing ICP’s on behalf of members and for use by PCP, Specialty Care Provider (SCP) and other community providers
- Coordinating and arranging for all services required by a member in accordance with the ICP
- Monitoring member’s health status and ensuring that member is receiving all necessary medical and supportive services
- Collaborating with other Case Managers to reduce variations in clinical practice, identify opportunities for systems’ improvement, and develop policies and procedures relevant to practice
We are looking for a Registered Nurse that is passionate about making sure every patient receives quality healthcare. You must have a high level of accountability and responsibility for outcome of care. The ability to work independently and maintain flexibility in fast paced start up environment is also essential.
- Additional requirements:
- Licensed Registered Nurse in appropriate State
- 5+ years of experience working with a geriatric population or equivalent experience
- Valid driver’s license
- Knowledge of Medicare and Medicaid programs, a plus
- Strong clinical and assessment skills
- Computer skills
- Ability to analyze data and use it to improve care delivery
- Highly organized with ability to manage multiple priorities appropriately
- Ability to work collaboratively and build enduring relationships with providers, members and the multidisciplinary team
- Ability to travel 50% or more of the time within service area as well as ability and willingness to cover on-call 24-hours per/day during specific periods
