Expanding home care agency has a position for Service Coordinator.  Position requires maturity, the ability to problem-solve and advocate for people in the Traumatic Brain Injury and Nursing Home Transition and Diversion Waiver Programs while working as part of a team.  Excellent verbal, oral and written communication skills are needed.  Computer literacy necessary.

Job Requirements

Growing homecare agency seeking a full time Service Coordinator with at least one year of experience for a full time position. Position located in Kingston, NY office with local travel to participants homes.  Milage reimbursement.

Additional position for Service Coordinator Supervisor.  Candidate for Supervisor position must have at least 1 year experience in either TBI or NHTD Waiver as a Service Coordinator

Will consider RN with recent Case Management experience
