Job Description

Provides skilled nursing care for assigned patients in home care environment. Assesses, plans, evaluates, and documents the home care nursing process as needed. Assesses the patients condition and the home environment. Provides health care instruction to patient and family members.

PRINCIPAL RESPONSIBLITIES Includes the following. Other duties may be assigned.

Respect each patients rights during the provision of care or service and conduct business in an ethical manner.

Positively represent the company to both internal and external customers maintaining a professional and courteous manner at all times.

Uphold the Alere core values in the conduct of work.

Assess patients prior to initiation of home care service for appropriateness of services prescribed and discuss concerns or questions with appropriate supervisor or manager.

Educate patients and families with a systematic approach, promoting patient understanding of treatment and services, health status, coping ability and patient/family involvement in the continuum of care or service.

Initiate the plan of treatment/care pathway based upon the analysis of information obtained.

Provide home care services according to Alere policies and procedures and prescriber plan of treatment.

Individualize patient assessments based on care/services provided and patient need.

Coordinate care with the primary center to promote the continuum of care or service.

Accurately communicate in timely manner patient status information to prescribing physician and provide necessary clinical information to Alere pharmacists and/or other healthcare disciplines as necessary, e.g. social workers, dieticians, etc.

Modify the plan of care, as appropriate, based upon reassessment, change in prescriber orders, the patients need for further care or services and the achievement of identified goals.

Ensure interventions respect and encourage the patients ability to make choices.

Provide home care services in an efficient, organized manner, handling the case load to optimize patient care needs and plan for geography. Maintain flexibility in assignments and scheduling as priorities may change throughout the course of the workday.

Complete clinical record documentation per Alere policies in a timely, complete and accurate manner.

Provide clinical support to key physicians/hospitals through personal contacts and in services, as directed.

Maintain competency on all services provided according to Alere policies.

Participate in all required continuing education, staff meetings, competency training and assessments.

Maintain all other personnel requirements as outlined per Alere policies.

Participate in continuously measuring, assessing and improving performance of clinical and other processes within the center.

Assume on-call responsibility as assigned. Participate in holiday and weekend schedule as assigned.

Job Requirements

EDUCATION/EXPERIENCE

BSN preferred

Minimumone year high risk perinatal, antepartum, labor and delivery or obstetrical nursing

Evidence of independent practice in delivering nursing care required

CERTIFICATES, LICENSES, REGISTRATIONS

Registered Nurse with current nursing license.

TECHNICAL SKILLS

Basic computer functionality such as the use of the intranet and email systems

COMMUNICATIONS SKILLS

Excellent written and verbal communication skills.

o Effectively communicate by telephone and writing.

o Interpersonal skills to effectively communicate with patients and other members of the health care team.

o Ability to document patient care clearly and accurately.

TRAVEL DEMANDS

Routine day and/or night travel require in all weather conditions. Most travel is same day; minimal overnight travel.

ON-CALL

Ability to work on-call as required. (Needs will vary by location)
