Manager of Care Coordination/Case Management at New York

Our Health Homes Care Coordination (HH) program coordinates medical, mental health and substance abuse services for Medicaid recipients. We are seeking a Manager of Care Coordination to provide clinical and administrative oversight to a team of supervisors responsible for care coordination and outreach activities, ensuring that program targets are met and policies and procedures are upheld. Primary work site location could vary based on the needs of the program; travel requires public transportation.

  • Provides supervision to a team of supervisors who are responsible for clinical/administrative activities of Care Coordinators (Case Managers) and Outreach staff; Leads/co-leads team meetings and HH manager’s meetings. 
  • Oversees department-wide daily operations, managing on and off-site clinical/administrative activity; oversees client flow, making recommendations as needed to retain client volume and provide clients with appropriate levels of care.
  • Manages the provision of appropriate clinical services and oversees their accuracy and timeliness.
  • Submits billing data, tracking sheets and other reports as requested by managers or funding sources.
  • Monitors employee performance standards for assigned teams and provides feedback.
  • Works collaboratively with staff to identify, design, and implement new initiatives; including systems and initiatives for specialized projects e.g., specialized trainings, outreach activities, etc.
  • Assists Director(s) in developing services tailored to meet the needs of NADAP’s clients. 
  • Develops, updates, and implements procedures to support operations at all sites.
  • Develops and maintains collaborative working relationships between referral sources including Health Homes and community programs to ensure that all program expectations are met.  Establishes relationships with community partners to promote NADAP services.
  • Performs QA reviews on care coordination/outreach files to ensure that all assessments, notes, care plans and consents are completed in a timely manner and within the P&P of the regulating HH program. Conducts utilization reviews to ensure clients are appropriately linked and engaged in needed programs.
  • Serves as liaison to community-based partners for issues related to referral and care coordination.
  • Conducts site visits to programs to ensure that clients’ needs are met. Represents NADAP in public forums/meetings.
  • Recruits, hires and trains staff, special project consultants, and student interns.
  • Ensures that staff are appropriately trained to and using Electronic Health Record systems, Health Home database, and any other relevant software.



Job Requirements

  • Minimum of three (3) years of direct supervisory experience to mid-level managers
  • Minimum of three (3) years of job-related experience providing medical, mental health or substance abuse-focused care coordination services to individuals with chronic medical conditions or severe and persistent mental illness
  • Education:  Master’s degree in social services or behavioral health field from an accredited university/college required


Required Skills

  • Excellent interpersonal, organizational, writing & computer skills
  • Strong knowledge of health care environments, clinical terminology and health information systems strongly preferred
  • Bilingual Spanish, Russian, or Chinese a plus
