Job Description
This Nurse Practitioner position provides medical services for residential and ambulatory persons in substance abuse treatment.
ESSENTIAL RESPONSIBILITIES:
- Conducts diagnostic services, order and review of laboratory tests, medical admission screening
- Prescribe, coordinate, delegate and evaluate care, including prescription of medicines listed in the approved institutional formulary.
- Participate in development and on-going activity with medical treatment plans and also QA activities.
- Performs other related duties as assigned by appropriate authority.
- Thorough understanding of clinic workflows.
- Demonstrates ability to handle emergency situations in a prompt, professional manner.
- Provides personal patient care to provide comfort and well being to clients by acknowledging physiological and psychological needs.
- Demonstrates safe handling in the cleaning, use and disposal of equipment and supplies.
- Makes recommendations for resident/staff education and training and coordinates provision of staff education.
- Assess clients by collecting and documenting the clients current health status and medical history.
- Plan and implement treatment and referrals. Perform all appropriate follow-up, schedule off-site treatment, and monitor clients until the condition is resolved.
- Utilize knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative care of assigned patient/family as evidence by documentation and observation of positive patient care outcomes.
- Perform delegated independent functions using established procedures, policies, guidelines, and standards.
- Obtains a complete medical psychiatric history including history pertaining to HIV risks. Make appropriate recommendations for testing and assessment if clients understanding and knowledge of HIV issues.
- Obtain required blood and urine samples. Prepare testing, and review results with MD, repeat as needed.
- Perform appropriate follow-up after taking history, including tuberculin skin test, schedule any off site out-patient appointments, and continue to monitor patients condition/sickness until resolved on subsequent clinic visits.
- Evaluate and perform on-going assessments and revise treatment plans as needed.
- Document nursing intervention, care provided, treatment needed, problems, limitations, and progress.
- Demonstrate ability to handle emergency situations in a prompt, professional manner.

- Counsel, train and teach health care related subjects to clients, family members and other staff members.
- Demonstrate safe handling in the cleaning, use and disposal of equipment and supplies.
- Maintain a clean, neat and safe environment for Clients. Ensure that supplies and equipment necessary for patient care are available and stored in a safe and organized manner.
- Additional duties as requested by supervisor
KNOWLEDGE, SKILLS & ABILITIES:
- Ability to read and comprehend instructions, medical correspondence and memos.
- Ability to write medical assessments, and correspondence.
- Ability to effectively present information in one-on-one and small group situations.
- Ability to draw blood
- Always be courteous to clients, families, and other staff members
- Excellent oral and written communication skills
- Adaptable to change in the work environment
Job Requirements
EDUCATION, CERTIFICATIONS & EXPERIENCE:
- Licensed Nurse Practitioner in New York State
- Masters Degree in Nursing with Approved Nurse Practitioner Program
- Experience in Substance abuse treatment is desirable.
- Medicaid Enrollment number is mandatory and essential
