The Home Care Clinical Nurse Specialist provides nursing services and care to Home Care patients in their place of residence. Responsibilities include providing assessments, care planning, and service coordination, along with providing direct care to each patient and instruction/supervision to HHAs, PCAs and other caregivers. Ideal incumbents will poses an in-depth knowledge regarding current nursing treatment standards and practices as well as a willingness to have a flexible schedule that accommodates patient needs. Previous work with adults is required, with previous nursing experience in med-surg or a home care setting strongly preferred.

Must possess a valid NYS Nursing license and registration.

Job Requirements

Associates degree is required, bachelor’s degree preferred. Position requires current driver’s license and reliable transportation.  UAS certified, PRI screen preferred. English/Spanish a must.

Serves as the on-call RN as needed.
